Cause of borderline personality disorder (BPD) is unknown. Genetic, family, and social factors are thought to play roles.

Risk factors for BPD include:

A person who is looked up to one day may be looked down on the next day. These suddenly shifting feelings often lead to intense and unstable relationships.

Other symptoms of BPD include:

Chronic Bronchitis

Simple chronic bronchitis is a condition in which there is chronic or recurrent increase in the volume of mucoid bronchial secretion sufficient to cause expectoration. Throat cleaners who swallow their sputum should also be included in this definition, but it is not possible to diagnose chronic bronchitis with confidence in the presence of other cardio-pulmonary disease which may themselves give rise to expectoration.

Chronic bronchitis may be complicated by infection, giving rise to chronic or recurrent muco-purulent bronchitis in which the sputum is persistently muco-purulent. On the other hand chronic bronchitis must be further complicated by persistent widespread narrowing of the airways at least on expiration which causes airways obstruction. It may then be termed chronic obstructive bronchitis.

Causes:

The following may be causes of this condition.

• Cigarette smoking: The death-rate among cigarette smokers is generally high.
• Atmospheric Pollution: Smokes and sulphur dioxide are the major factor in the air pollution.
• Infection: Infection with respiratory Bacteria’s (Mycoplasma pneumonia) and viruses especially H. influenza causes most of the danger.
• In occupations, dust and fumes e,g, in coal mines and steel and flax workers, there is greater prevalence of the disease.
• Chronic bronchitis is more common in men and incidence increases with age.
• Infection increases air way obstruction and poor ventilation and decrease in blood flow, the condition deteriorates further.

Clinical picture:

• The onset is extremely insidious; throat clearing and the swallowing may be the only symptoms left on the surface.
• Usually there is expectoration of mucus with cough after waking.
• As age advances, the cough becomes more troublesome. The sputum is at first mucoid, but in many contains black specks of carbon particles from smokes. Later it may become yellow or green.
• Haemoptysis is rare.
• Breathlessness may first become apparent very gradually on exertion and then it may be accompanied by wheeze.
• There may be no physical sign but with increasing mucus secretion, rhonchi may be heard which clear on coug
• Airway obstruction increase and with it wheezing may become persistent.
• Clubbing of fingers is exceptional and should give rise to suspicious that carcinoma of the lung is present.
• Advanced chronic bronchitis leads to respiratory failure. There may be loud closure of pulmonary valve.
• The liver is enlarged and there is peripheral oedema.

Diagnosis:

 It is made by examining the respiratory movements of the chest, there rate and rhythm etc.

Complication:

Asthma is often associated. In long-standing case, bronchiectasis, emphysema, and ultimately congestive heart failure may appear. From frequent coughing, hernia may develop.

Treatment: General

• The patient should be put to bed.
• Dusty, stuffy and damp atmosphere should be avoided.
• During the attack, no fats should be used, and fresh luke-warm water be drunk.

Psoriasis is a common skin disease that affects the life cycle of skin cells. Psoriasis causes cells to build up rapidly on the surface of the skin, forming thick silvery scales and itchy, dry, red patches that are sometimes painful.

The patches range in size from small to large. They most often appear on the knees, elbows, scalp, hands, feet, or lower back. Psoriasis is most common in adults. But children and teens can get it too.

Having psoriasis can be embarrassing, and many people, especially teens, avoid swimming and other situations where patches can show. But there are many types of treatment that can help keep psoriasis under control.

Types of Psoriasis

Psoriasis appears in a variety of forms with distinct characteristics. Typically, an individual has only one type of psoriasis at a time. Generally, one type of psoriasis will clear and another form of psoriasis will appear in response to a trigger.

Plaque Psoriasis (Psoriasis Vulgaris)

• Plaque psoriasis is the most common form of the disease and appears as raised, red patches covered with a silvery white buildup of dead skin cells or scale. These patches or plaques most often appear on the scalp, knees, elbows and lower back. They are often itchy and painful, and they can crack and bleed.

Nail psoriasis.

• Psoriasis can affect fingernails and toenails, causing pitting, abnormal nail growth and discoloration. Psoriatic nails may become loose and separate from the nail bed (onycholysis). Severe cases may cause the nail to crumble.

Scalp psoriasis.

• Psoriasis on the scalp appears as red, itchy areas with silvery-white scales. You may notice flakes of dead skin in your hair or on your shoulders, especially after scratching your scalp.

Guttate

• Guttate psoriasis is a form of psoriasis that often starts in childhood or young adulthood. This is the second most common type of psoriasis, after plaque psoriasis. About 10 percent of people who get psoriasis develop guttate psoriasis.

Inverse

• Inverse psoriasis also known as inter triginous psoriasis, shows up as very red lesions in body folds. It may appear smooth and shiny. Many people have another type of psoriasis elsewhere on the body at the same time.

Pustular

• Pustular psoriasis in characterized by white pustules (blisters of noninfectious pus) surrounded by red skin. The pus consists of white blood cells. It is not an infection, nor is it contagious.

Erythrodermic

• Erythrodermic psoriasis is a particularly inflammatory form of psoriasis that often affects most of the body surface. It may occur in association with von Zumbusch pustular psoriasis. It is a rare type of psoriasis, occurring once or more during the lifetime of 3 percent of people who have psoriasis. It generally appears on people who have unstable plaque psoriasis. This means the lesions are not clearly defined. Widespread, fiery redness and exfoliation of the skin characterize this form. Severe itching and pain often accompanies it.

NOTE: Individuals having an erythrodermic psoriasis flare should see a doctor immediately.This form of psoriasis can be life-threatening.

Psoriatic arthritis.

• In addition to inflamed, scaly skin, psoriatic arthritis causes pitted, discolored nails and the swollen, painful joints that are typical of arthritis. It can also lead to inflammatory eye conditions, such as conjunctivitis. Symptoms range from mild to severe, and psoriatic arthritis can affect any joint. If you have psoriasis, however, the T cells attack healthy skin cells by mistake, as if to heal a wound or to fight an infection.

Other factors may contribute to the development of psoriasis, make the condition worse, or make it return, including:

• Climate. Cold, dry weather causes symptoms to become worse. Hot weather, sunlight, and humidity may improve symptoms.
• Skin injury. An injury to the skin can cause psoriasis patches to form anywhere on the body, including the site of the injury. This includes injuries to your nails or nearby skin while trimming your nails.
• Stress and anxiety. Stress can cause psoriasis to appear suddenly (flare) or can make symptoms worse.

What is ADHD?

Attention deficit hyperactivity disorder (ADHD) and attention deficit disorder (ADD) refers to a range of problem behaviours associated with poor attention span.

• Disorganised about tasks and activities.
• Avoids tasks like homework that require sustained mental effort.
• Loses things necessary for certain tasks or activities, such as pencils, books or toys.
• Easily distracted.
• Forgetful in the course of daily activities.

Hyperactivity

A child must have exhibited at least three of the following symptoms for at least six months to an extent that is unusual for their age and level of intelligence.

• Runs around or excessively climbs over things. (In adolescents or adults only feelings of restlessness may occur.)
• Unduly noisy in playing, or has difficulty in engaging in quiet leisure activities.
• Leaves seat in classroom or in other situations where remaining seated is expected.
• Fidgets with hands or feet or squirms on seat.

Impulsivity

At least one of the following symptoms must have persisted at least for six months to an extent that is unusual for their age and level of intelligence.

• Blurts out answers before the questions have been completed.
• Fails to wait in lines or await turns in games or group situations.
• Interrupts or intrudes on others, e.g. butts into others conversations or games.
• Talks excessively without appropriate response to social restraint.

What causes ADHD?

Biological factors

• The child’s temperament, as this contributes to their attitude and personality.
• Studies of twins suggest a genetic link to ADHD. In 80-90 per cent of identical twins where one has ADHD so does the other. Recent research also suggests there is a greater chance of inheriting the condition from male relatives such as grandfathers.
• Brain injuries due to birth trauma or pre-birth problems. The brain structures believed to be linked to the development of ADHD are vulnerable to hypoxic damage during birth. The damage is caused by inadequate oxygen reaching parts of the brain while blood flow is reduced.

Environmental factors

• Family stress.
• Educational difficulties.

How is ADHD diagnosed?

There is no single diagnostic test for ADHD so different sorts of information needs to be gathered, such as the following:

History of symptoms

The precise nature of the difficulties, when they were first noticed, in what situations they occur, factors that exacerbate or relieve them.

Medical history

Risk factors that could predispose the child to ADHD include difficulties and risks in pregnancy and during birth, for example if the mother was in poor health, very young or drank alcohol or smoked or had an extended or complicated labour.

This will include discussion of the methods used by the parents to manage the child’s behaviour and how successful they have been. Although this seems intrusive, the assessor will remain neutral and parents should not feel the disorder is ‘their fault’.

Family history

The mental and physical health of the child’s parents and other family members can be relevant, particularly regarding the incidence of ADHD or depression.

Social assessment

The family’s social circumstances, such as housing, poverty, and social support may all have an impact on the child’s behaviour.

Who can get ADHD?

Children of all backgrounds can have ADHD. Teens and adults can have ADHD too.

Causes of ADHD

No one knows for sure. ADHD probably comes from a combination of things. Some possibilities are:

• Genes, because the disorder sometimes runs in families

Or, a child may act badly in school, but teachers don’t realize that the child has ADHD.

Abdominal pain

Abdominal pain is pain that you feel anywhere between your chest and groin. This is often referred to as the stomach region or belly.

Considerations

Almost everyone has pain in the abdomen at one time or another. Most of the time, it is not caused by a serious medical problem.

How bad your pain is does not always reflect the seriousness of the condition causing the pain.

For example, you might feel severe abdominal pain if you have gas or stomach cramps due to a viral gastroenteritis.

Sometimes, life-threatening conditions such as colon cancer or early appendicitis may only cause mild pain or no pain.

Other ways of describing pain in your abdomen include:

• Pain may be generalized, meaning that you feel it in more than half of your belly. This is more typical for a stomach virus, indigestion, or gas. If the pain becomes more severe, it may be caused by a blockage of the intestines.
• Pain that is localized is found in only one area of your belly. This type of pain is more likely to be a sign of a problem in an organ, such as the appendix, gallbladder, or stomach.
• Cramp-like pain is usually not serious, and is more likely to be due to gas and bloating. It is often followed by diarrhea. More worrisome signs include pain that occurs more often, lasts than 24 hours, or occurs with a fever.
• Colicky pain is pain that comes in waves. It usually starts and ends suddenly, and is often severe. Kidney stones and gallstones are common causes of this type of belly pain.

Common Causes

Many different conditions can cause abdominal pain. The key is to know when you need to get immediate medical care. Sometimes you may only need to call a doctor if your symptoms continue.

Less serious causes of abdominal pain include:

Home Care

If you have mild abdominal pain, the following tips might be helpful:

• Sip water or other clear fluids.
• Avoid solid food for the first few hours.
• If you have been vomiting, wait 6 hours, and then eat small amounts of mild foods such as rice, applesauce, or crackers. Avoid dairy products.
• If the pain is high up in your abdomen and occurs after meals, antacids may help, especially if you feel heartburn or indigestion. Avoid citrus, high-fat foods, fried or greasy foods, tomato products, caffeine, alcohol, and carbonated beverages.
• Avoid aspirin, ibuprofen or other anti-inflammatory medications, and narcotic pain medications unless your health care provider prescribes them. If you know that your pain is not related to your liver, you can try acetaminophen (Tylenol).

Prevention

The following steps may help prevent some types of abdominal pain:

• Avoid fatty or greasy foods.
• Drink plenty of water each day.
• Eat small meals more frequently.

• Limit foods that produce gas.

AIDS

AIDS stands for: Acquired Immune Deficiency Syndrome

AIDS is a medical condition. A person is diagnosed with AIDS when their immune system is too weak to fight off infections.

Cause

AIDS is caused by HIV.

HIV is a virus that gradually attacks immune system cells. As HIV progressively damages these cells, the body becomes more vulnerable to infections, which it will have difficulty in fighting off. It is at the point of very advanced HIV infection that a person is said to have AIDS. If left untreated, it can take around ten years before HIV has damaged the immune system enough for AIDS to develop.

The virus can be spread (transmitted):

• Through sexual contact — including oral, vaginal, and anal sex
• Through blood — via blood transfusions (now extremely rare in the U.S.) or needle sharing
• From mother to child — a pregnant woman can transmit the virus to her foetus through their shared blood circulation, or a nursing mother can transmit it to her baby in her breast milk

HIV infection is NOT spread by:

• Casual contact such as hugging
• Mosquitoes
• Participation in sports
• Touching items that were touched by a person infected with the virus

People at highest risk for getting HIV include:

• Injection drug users who share needles
• Infants born to mothers with HIV who didn’t receive HIV therapy during pregnancy
• People engaging in unprotected sex, especially with people who have other high-risk behaviours, are HIV-positive, or have AIDS
• People who received blood transfusions or clotting products between 1977 and 1985 (before screening for the virus became standard practice)
• Sexual partners of those who participate in high-risk activities (such as injection drug use or anal sex)

Symptoms

AIDS begins with HIV infection. People who are infected with HIV may have no symptoms for 10 years or longer, but they can still transmit the infection to others during this symptom-free period. If the infection is not detected and treated, the immune system gradually weakens and AIDS develops.

Almost all people infected with HIV, if they are not treated, will develop AIDS. There is a small group of patients who develop AIDS very slowly, or never at all. These patients are called non progressors, and many seem to have a genetic difference that prevents the virus from significantly damaging their immune system.

The symptoms of AIDS are mainly the result of infections that do not normally develop in people with a healthy immune system. These are called opportunistic infections.

People with AIDS have had their immune system damaged by HIV and are very susceptible to these opportunistic infections.

Common symptoms are:

• Chills
• Fever
• Sweats (particularly at night)
• Swollen lymph glands
• Weakness
• Weight loss

Note: At first, infection with HIV may produce no symptoms. Some people, however, do experience flu-like symptoms with fever, rash, sore throat, and swollen lymph nodes, usually 2 – 4 weeks after contracting the virus. Some people with HIV infection stay symptom-free for years between the times when they are exposed to the virus and when they develop AIDS.

Signs

CD4 cells are a type of T cell. T cells are cells of the immune system. They are also called “helper cells.”

Common with a CD4 count below 350 cells/mm3:

• Herpes simplex virus — causes ulcers/small blisters in the mouth or genitals, happens more often and usually much more severely in an HIV-infected person than in someone without HIV infection
• Herpes zoster (shingles) — ulcers/small blisters over a patch of skin, caused by reactivation of the varicella zoster virus, the same virus that causes chickenpox
• Kaposi’s sarcoma — cancer of the skin, lungs, and bowel due to a herpes virus (HHV-8). It can happen at any CD4 count, but is more likely to happen at lower CD4 counts, and is more common in men than in women.
• Non-Hodgkin’s lymphoma — cancer of the lymph nodes
• Oral or vaginal thrush — yeast (typically Candida albicans) infection of the mouth or vagina
• Tuberculosis — infection by tuberculosis bacteria mostly affects the lungs, but can also affect other organs such as the bowel, lining of the heart or lungs, brain, or lining of the central nervous system (brain and spinal cord)

Common with CD4 count below 200 cells/mm3:

• Bacillary angiomatosis — skin sores caused by a bacteria called Bartonella, which may be caused by cat scratches
• Candida esophagitis — painful yeast infection of the oesophagus
• Pneumocystis jiroveci pneumonia, “PCP pneumonia,” previously called Pneumocystis carinii pneumonia, caused by a fungus

Common with CD4 count below 100 cells/mm3:

• AIDS dementia — worsening and slowing of mental function, caused by HIV
• Cryptococcal meningitis — fungal infection of the lining of the brain
• Cryptosporidium diarrhoea — Extreme diarrhoea caused by a parasite that affects the gastrointestinal tract
• Progressive multifocal leukoencephalopathy — a disease of the brain caused by a virus (called the JC virus) that results in a severe decline in mental and physical functions
• Toxoplasma encephalitis — infection of the brain by a parasite, called Toxoplasma gondii, which is often found in cat feces; causes lesions (sores) in the brain
• Wasting syndrome — extreme weight loss and loss of appetite, caused by HIV itself

Common with CD4 count below 50/mm3:

• Cytomegalovirus infection — a viral infection that can affect almost any organ system, especially the large bowel and the eyes
• Mycobacterium avium — a blood infection by a bacterium related to tuberculosis

There is no cure for AIDS at this time. However, a variety of treatments are available that can help keep symptoms at bay and improve the quality of life for those who have already developed symptoms.
